|A fire extinguisher, and carbon monoxide and smoke detectors were observed. The smoke detector was tested and was operational. Earthquake and Fire drill was last conducted on 09/08/2017. The Licensee stated that she does not transport children at this time but she understands that children cannot be left in parked vehicles unattended at any time.|
Childrenâ€™s files were reviewed, which included records of receipt for Parents' Rights and Immunization. Safe sleep information and materials for infants were reviewed and provided to Licensee.
Licensee files were reviewed, which included records of Criminal Record and Child Abuse Index Clearance or Exemption, TB clearance, Statement Acknowledging Requirement to Report Suspected Child Abuse, and required Training. Licensee Pediatric First Aid/CPR/AED Certificates expire July 2019. Licensee stated that she currently has six children enrolled at her child care home; therefore, she does not need an Assistant Provider. Licensee understands her capacity options and she understands that she cannot have more than 14 children in the home at any time. Licensee also understands that she must comply with the ratio and capacity requirements of a small Family Child Care Home license whenever she is alone with the day care children.
Licensee stated that she has Measles and Pertussis vaccinations. Licensee had declined the flu shot. Licensee will obtain a copy of the immunization documents and maintain them in the personnel files and send a copy to Licensing Office within 30 days.
Adults who are over the age of 18 and reside in the home are the Licensee, and Licensee's husband, and Licensee's mother in law. They have Criminal Record Clearance and TB clearance.
LPA reminded Licensee of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license, and who come in contact with or provide care and supervision to the children. For an initial violation, civil penalty amounts to $100.00 per person per day with a minimum of $100.00 to a maximum of $500.00 per person. For a subsequent violation within a 12-month period, civil penalty amounts to $100.00 per person per day with a minimum of $100.00 to a maximum of $3000.00 per person.
