|(3) Licensing Program Analyst Darlene Bossu (LPA) conducted an unannounced facility visit. Licensee was present with 6 day care children in which 2 were infants. LPA stated the reason for the visit. LPA toured and no unsafe conditions were viewed inside or outside the facility. All required forms are posted. Licensee has completed all required forms for her facility. |
Licensee has a working smoke alarm and carbon monoxide detector. Fire ext. was charged August 2015 and will be charged this August as required. Licensee's CPR/First Aid card expires 6/2017.
LPA reviewed SB 277 new law - children's immunization's. Licensee was advised to visit www.shotsforschool.org for Immunization information on new law SB 277. LPA also reviewed SB 792- immunization's for staff. Quarterly update was given to the licensee today.
THE NOTICE OF SITE VISIT WAS POSTED BY LPA AS REQUIRED BY H&S CODE SEC. 1596.817. The NOTICE OF SITE VISIT MUST REMAIN POSTED FOR 30 DAYS OR A CIVIL PENALTY OF $100.00 WILL APPLY.
All individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Failure to obtain a criminal record background check clearances prior to initial presence at the facility will result in an immediate $100.00 dollar civil penalty per day for the first 5 days. 2nd violation within 1 year will result in a civil penalty of $100.00 per day for 30 days.
