1, LPA Ng conducted an unannounced random annual visit. LPA met with Kayla Lee, Director and Ellen Park during the visit. Facility is a preschool program center. Rooms 1,3,4,5 and 7 are used for the preschool program. There were 25 children with 5 staff members present.
CLASSROOMS: . Furniture and equipment are age appropriate and in good repair. There is adequate heating, lighting and ventilation. Drinking water is readily available via water pitchers and paper cups there is sufficient storage available for the children's belongings and sufficient space for the group size being served. There are separate toilet and sink facilities available for staff & children. There are sufficient napping equipment for each child. Proper staffing ratios are maintained. Director states that there are no known guns of weapons in the facility.
BATHROOMS AND TOILETING AREAS: Toilets flush. Faucets work. Toilets and sinks are reachable by children. Toilet paper, soap and towels are available and the bathrooms are clean. There is sufficient lighting and ventilation. Hot water was tested at 82.5 degrees F.
INSPECTION OF FOOD SERVICE AREA: The school provides both the morning and afternoon children 2 meals one lunch, and one snack to the children. The food is prepared on site. There is a food prep kitchen area as well as a built in cafe where parents can sit down for food in the facility, the food was inspected for freshness and all items were in compliance. The menu was posted
INSPECTION OF OUTDOOR PLAY AREA: There are ample toys and activities for children. The facility has an outdoor area. The outdoor area has a play structure, the structure was in good condition. The flooring material is of a soft rubber surface. The facility also enhances its outdoor space with a gross motor space. The gross motor space was observed to be in good condition and the floor is padded. This facility also has a large gross motor space which is scheduled use. The children are not comingled.

HEALTH-RELATED SERVICES: Site Director states medications will not be dispensed except those items that are used for emergencies such as epi-pens, benydrl, nebulizers, etc.) parents will provide note if there is such a case. Applicant states that medication will be stored in Lock boxes at the front desk and at the cafe refrigerator. Children will be greeted and checked for illness before entry into the facility. First aid supplies and thermometer are available. There are emergency items available. Isolation is provided in the office with the use of the Plaza bathroom which is located on the 3rd level, if needed.
RECORDS: All of the children's records were complete. Facility has posted the required forms (i.e. License, menus, waivers, Notification of Parent's Rights, Notification of Personal Rights, Car Seat Law, and Emergency Disaster Plan). Staff persons are current with First Aid/CPR certification expiring May 2016. Site Director states Fire and Earthquake drills are done each month, last logged 04/15/15.
DISCIPLINE POLICY: Director states positive reinforcement and redirection.
