(2) LPA Monica Cuddy visited the facility to conduct a site inspection. This is a Title V state preschool program housed in room 34 at Pioneer Elementary school. Upon arrival LPA met with Lead Teacher Raquel Reyes and proceeded to tour the playground and classroom. During today's visit, there were 21 children with 3 staff in one classroom. Appropriate ratios and capacity were observed. Appropriate care & visual supervision were also observed during the visit while children were having morning snack.
Furniture and age appropriate equipment is in good condition. The room has adequate heating, lighting, ventilation. Drinking water is readily accessible. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of the school cafeteria. Menu is posted. Adequate food is available for meals. Cleaning supplies are kept separate from food. Storage containers for solid waste have tight-fitting covers and are kept in good repair. The facility appears to be free of insects and rodents.
Outdoor play area is the kindergarten playground with sufficient material for cushioning. There are no bodies of water or weapons at this facility. Climbing structures, swings and slides are securely fixed to the ground. Area has trees used for shade. Equipment is age appropriate. Area has drinking water readily accessible and grounds are free of debris or potential hazards.
All medications are kept and administered through the school nurse. Personnel records contain documentation of education and children’s records contain emergency information. At least one staff member has current CPR and First Aid certifications. All staff must meet educational requirements of Department of Ed and are required to complete medical exam and TB tests as a condition of employment. Staff is also exempt from criminal record clearance requirements as employees of the Board of Education. Incidental Medical Services were discussed. LPA provided notice of site visit and observed it being posted at the facility. The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. LPA requested up to date copies of Personnel Roster (LIC500) and Emergency Disaster plan (LIC610) be sent to licensing. No deficiencies are cited.
