Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced annual random inspection. Upon arrival LPA rang the doorbell/buzzer at the front gate that reads, "Please Ring Buzzer." LPA rang the doorbell and there was no answer. LPA called the telephone number on file and was informed that the number was for Whittier Head Start and was then provided with the number 562-427-3424. LPA called in order to enter the facility. LPA met with Sophia Johnson, Head Teacher who provided the tour of the facility. LPA was later met by Blanca Litzinger, Early Learning Center Manager and Jennie Acosta, Program Administrator and they were also advised about the telephone number on file. The Head Teacher guided analyst on a tour of the facility. This is a preschool program which consists of 3 classrooms; Rooms B1, B2, and B3. Facility operation hours are Monday to Friday. Room B2 operates 2 sessions. The AM session is from 7:45AM to 11:15AM and the PM session is from 12:15PM to 3:45PM. In Room B1 there is an extended day program that operates from 8AM to 3:45PM. Room B3 is currently being used as an office. LPA advised that the facility is licensed for 60 preschoolers which includes Room B3, and the facility needs to submit an updated application form to reflect the new telephone number and if they are planning to reduce capacity.

All areas identified on the report were inspected. Upon arrival, the following staff were present during this inspection: Room B1: Staff #1 and #2 with 20 napping preschoolers, and Room B2 Staff #3 and #4 with 17 preschoolers. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. LPA advised the Head Teacher to make sure that children's belongings are not touching one another. Linens are washed weekly by a laundry service. Napping equipment (cots) were observed to be utilized by the napping children in Room B1. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Head Teacher states that there are no poisons stored at the facility.

There is no kitchen or food preparations area at the facility. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition, however, upon inspecting the fence, LPA observed that there is a gated door that leads to the staff parking lot. The door was closed but not locked. LPA advised the Head Teacher that the gated door needs to be locked to prevent children from having access to the staff parking lot. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed.

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed for completeness; Inspection of required forms was made. LPA issued the Children’s Record Review (LIC 857) to the Head Teacher during this inspection. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

In review of staff records, LPA observed that Staff #2 and Staff #3 do not have the Mandated Reporter Certificate on file. LPA also issued the Review of Staff records (LIC 859) to the licensee during this inspection. The LIC 857 and the LIC 859 documents the staff and children’s files that were reviewed during this inspection. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 12/18.

Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. Preschoolers in the AM session are provided with a breakfast. Children from the PM are provided with a lunch. Children from the extended day program are provided with breakfast, lunch, and snack. All meals and snacks are provided by the Long Beach Unified School District and are delivered daily to the facility.

First Aid supplies were observed in each classroom. According to the Head Teacher, medication is only administered to a child when accompanied with a doctor's note and they are stored in locked medicine boxes in each classroom.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA advised licensee of the Mandated Reporter Child Care Training that became in effect January 1, 2018, and to maintain a copy of the certificate on file for each staff.

The following deficiencies listed on the attached deficiencies page are being cited in accordance with California Code of Regulations Title 22.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Blanca Litzinger, Early Learning Center Manager, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. ----Page 3 of 3

Outdoor Activity Space. Outdoor activity space shall be hazard free.
This requirement is not met as evidenced by LPA observing that the gated door along the fence that leads to the staff parking lot was not locked. This poses a potential health and safety rick to children in care.
